Insurance Coverage for Ancillary Services
No service charges will be submitted to insurance coverage through Illinois Bone and Joint’s billing department. If a client believes their insurance plan will cover the cost of services; the client must make full payment at the time of service and personally submit the claim to their insurance provider. Any monies received in form of reimbursement must be submitted back to the client and not to Illinois Bone and Joint Institute, LLC.
24-Hour Cancellation Policy for Services
There is a 24-hour notice for all cancellations or rescheduling of appointments, without exception. Please be aware that failure to do so will result in the full charge of the session or loss of the session from a previously purchased package.
Credit Card Authorization
Illinois Bone and Joint Institute is capable of utilizing a stored credit card used for a previous purchase to process charges for services moving forward including monthly membership charges.
Membership Terms and Conditions
I understand that Illinois Bone & Joint Institute reserves the right to, among other items,: (i) change monthly dues without notice, (ii) change or eliminate facilities, services, or programs, or (iii) change its hours of operation. I understand that Illinois Bone & Joint Institute reserves the right to revoke or suspend my membership, without refund, without notice at any time if I fail to follow any rules or regulations, conduct myself in a manner which management deems inappropriate or disruptive to other Illinois Bone & Joint Institute members or staff or make false representation of information contained in this Agreement, whereupon I will return my membership card to Illinois Bone & Joint Institute. I understand that if I terminate my membership before the end of the month or the package I have purchased expires, all fees will be non-refundable. These policies help us provide the highest quality of service to all our participants.
Use of Group Sessions
All packages expire December 31, 2025 without exception. All first sessions must be redeemed by January 31, 2025.
Use of Private Sessions
All private packages expire December 31, 2025 without exception. Private sessions MUST be used in the allotted week. If any sessions in a week are missed, no make-up session will be provided. Private sessions are not shareable and are for individual use only. Sessions cannot be split between multiple family members or friends. Credits: (1) Unused packages can be substituted for other services at full retail value; (2) Expiration dates apply to credit transfers.

Class times are subject to change month to month.
Refunds
All programs are non-refundable unless services cannot be provided by the IBJI Health Performance Institute due to Covid-19 or other "Act of God." In the event of a mandated closure, all IN-PERSON training and/or WELLNESS services that have been purchased will carry over once the government allows gyms to resume in-person training. No "missed" sessions due to a MANDATED CLOSURE will be lost.
Credits
In the event a package cannot be used, clients may substitute their purchase package for any other services but must purchase exchanged services at full retail value. Expiration dates still apply to credit transfers.
